Where health care providers sought the dismissal with prejudice of a medical malpractice action based on the plaintiff’s failure to timely file a health care affidavit, the plaintiff’s affidavits failed to identify the medical expert and to provide his address and qualifications as required by statute, so the trial court is ordered to dismiss the action.
